Couple Question about Tyr's Hand - Please reply!

Alright so I've made my decision, I'm going to purchase the tyr's hand rapid fire controller because it's the cheapest and looks easy.

My questions are:

1. Does it come already all programmed like shown in the video on "youtube" by dakotakid12345
2. If it doesn't, how would I program it?
3. Does it also work with Call of Duty World at War? By that I mean does it not get mod blocked?
