I have no idea why but my new controller that I got today was being glitchy with rapidfire and I couldn't understand why:/. Every time I would hold rt my mode of controller would just reset?? So I went to setting changed the controller vibrations to off and it work perfectly now?? I guess vibration of rumblers glitches the chip up so if your having problems just do what I did.

Thats odd try brand new batteries and re-flashing the firmware to your controller. It doesnt sound like a firmware issue however I would give it a shot.

Just try new batteries. If your batteries say "original equipment accessory" and are duracell, throw them in the trash. If you are using a rechargeable pack, make sure it's charged for 24 hours before it's first use! Rechargeables need to be recharged, especially on their first use.
