Optional

It's so that you won't loose anything important in case it goes wrong or you have to rollback the service pack.

Of course a good backup is a wise thing to do but this is definitely optional.

So should I create a operating system backup or just the things I want?

Do something that will change your life forever.

Backup all your important files on DVD or USB flash drive. Then go purchase a copy of Windows 7, format your hard drive, and install Windows 7. You will be so amazed, your life will change before your very eyes, your computer will run faster, you will achieve fame, wealth and fortunes unimaginable.
